Reports and Dashboard by sales rep on following requirement

I have the following requirement to be done,in my analysis what i realised is this is the work of a summary report however I am facing the following issues

1)In my analysis as I understand this consists of the use of custom summary filed formulas,and as per the doc 5 is the limit,how to achieve the following in that case

2)How to get the coloums filtered accoording to indivisual stages in the same report,for examplet the 6th field from the left named Gross($) is only asking for the sum of Gross amounts to be displayed which have resulted in a closed Won,and in the same report the 8th field from the left named Stage=Cancelled(Cancelled$) is asking for the amount of cancelled $.how is this achievebe in a single report

3)What would be the best report type to be used in this case,I am using a ‘lead vs all converted items’ report type.
