Why was the fed discount rate lower than the effective federal funds rate before 2003?

As shown by https://fred.stlouisfed.org/graph/?g=zXIb the federal funds discount rate was mostly lower than the effective federal funds rate before 2003. In that case why didn’t all banks borrow from the fed discount window instead of from each other? Why did it suddenly switch from being lower to higher in Jan 2003?
