Cases where a noun with its adjective omitted has a different meaning to the noun with its adjective not omitted

In most cases,

Happy bird = bird
Beautiful car = car
Small chair = chair

But,

A half apple = not an apple
A double o = not an o
An almost hour = not an hour

Why do these cases happen, and are there any further examples?
